What information do you need to make plans for your discharge or the discharge of your family?

Discharge planning is a process which begins with the patient's admission and works cooperatively with the patient, physician and clinical staff to plan for a safe discharge to a location which permits the least confined level of care and the safest outcome.
Sutter Delta's Case Managers are available to assist you with your planning needs following a hospital episode. Case Managers are knowledgeable nurses and health care providers who are aware of the community resources and services, which will help you continue your recovery and maintain your health. Sutter Delta has several case managers to help you. Each is assigned by location of the patient. They are:

    • Robert Fabricante
    Social Worker

    • Carole Heapy
    CCU/Telemetry

    • Mike Whitehair
    Med/Surg - 3rd Floor

    • Carin Kaplan
    1st Floor Med Surg

    • Denise Maldonado
    VNA Float

    • Margie Huff
    Home Care

You can contact a Case Manager by calling the hospital at
    (925) 779-7200 and asking the operator for the specific Case Manager.

  • Pittsburg Care Center
  • (925) 432-3831

Other facilities are utilized in Central Contra Costa County depending on whether your physician will follow at the facility. These facilities are:

Insurance coverage is historically limited to those patients needing skilled nursing care as documented by standards of Medicare (Health Care Financing Administration). Traditionally, many persons are not aware that Medicare coverage for skilled care is not unlimited and often is for a short period. Each of the facilities that we have listed as Skilled nursing facilities, which our medical staff utilizes, will be happy to explain the limitations and the expected or anticipated costs involved in placement at their facility. If you are insured with an HMO form of insurance these choices may be limited to the facilities, which have contracts for care. You may wish to contact the medical group that is affiliated with your physician providing care. Historically, these groups are:

  • East County Medical Group
  • (925) 778-9272

  • Hill Physicians Medical Group
  • (925) 820-8300
